A Caregiver's Daily Journal and Organizer cover with flowers and a palm leaf, shown with sample pages. Printable PDF, 90 days, 140 pages.

We kept everything in a spiral notebook. Whoever was with Mom that day wrote down whatever came to mind. There was no order to it, and when I needed to look back at something, I couldn't find it. I made this journal so the next person who steps in knows what a good day looks like, what the medicines are, and how she was that day.

A Caregiver's Daily Journal & Organizer is a printable PDF for the gentle, unglamorous, sacred work of caregiving. Print it at home, punch holes or bind it your own way, and start today. There are no dates to fill in ahead of time, so you can begin any day and skip a day without losing your place.

What's inside (140 pages)

  • Why I Made This Journal (a note from me)
  • How to Use This Journal
  • How to Print, and Terms of Use
  • 12 reference pages you fill in once: Care at a Glance, Safety Plan, Current Medications, Doctors and Care Team, Who's Helping, Daily Rhythm, and more
  • 91 Daily Care Logs: sleep, meals and drinks, medications given, mood, bathroom, activity, visitors, nine lines for notes, and a "For the next caregiver" box
  • 13 Weekly Caregiver Check-Ins, just for you, not for them
  • 15 Mood & Behavior Logs for the harder moments: what was happening right before, what you noticed, what helped
  • 4 blank Notes pages

What you get

Two PDF files, so you can print the size that fits your printer: US Letter (8.5 x 11 in) and A4. Both files are the same 140-page journal, with flowers and a palm leaf on the pages and plum and gold touches on the headings.

How it prints

Print at 100% (actual size). The flowers print beautifully in color. To save ink, choose black and white or grayscale in your printer settings. Most home printers leave a thin white border at the page edge, which is normal. The left margin is a little wider so you have room to punch holes or bind. Print only what you need, as often as you need, for your own family.

After you buy

This is an instant digital download. No item is mailed to you. Your files are ready right after checkout. You'll find them on the thank-you page and in your order confirmation email. If you have any trouble, send me a message and I'll help.

Good to know

  • Digital download only. Nothing is shipped.
  • The PDF is not editable. It is made to be printed and written on by hand.
  • For personal use only. Please do not resell, share, or redistribute the files.
  • This journal is a record-keeping tool. It is not medical advice, and it does not replace your doctor or care team.
  • Colors on screen may look a little different from your printed pages.
